and Concussion Release Form
Volleyball and Football practice begins Monday, August 13, 2018. All JH and HS students must have a physical form completed each school year BEFORE they may practice. Click on the link to access the form. Students athletes must also have an updated Concussion Release Form signed by both the parent and the student on file each year. Copies of the forms will also be available in the HS Office.
